Home Health Physical Therapist

Job Description

Home Health Physical Therapist (PT)
South Franklin County & Fairfield County, OH | Full-Time

  • Set your own schedule, with no on-call and no weekends
  • Mileage reimbursement: $0.72 per mile
  • New grads and therapists new to home health are welcome

At National Church Residences, every role connects to one mission: enriching the lives of seniors and their families. In Home Health, that mission meets people where they most want to be, at home. As a Home Health PT, you’ll help older adults recover from illness, injury, surgery, or a hospital stay so they can stay safe and independent.

Why therapists choose Home Health with us

In a hospital or SNF, you might be caring for a whole hallway. Here, your full attention goes to one patient at a time, in their own environment, where you can see the whole picture: their routines, their caregivers, and the barriers that are easy to miss in a clinic. You’ll work independently in the field, but never alone. Nurses, OTs, SLPs, social workers, aides, and clinical leaders are behind you on every case.

What you’ll do

  • Evaluate strength, balance, and mobility, and build treatment plans around each patient’s real routines and goals
  • Help patients reach everyday wins, like managing the stairs, getting out of a favorite chair, or walking to the mailbox
  • Identify fall hazards and recommend equipment or simple changes that support independence
  • Teach patients and caregivers exercises and safe movement to carry progress between visits
  • Supervise PTAs and home health aides, and take part in case conferences and quality improvement
  • Document visits and coordinate with the care team using mobile technology

Room to grow
We promote from within, and many of our leaders started in the field. Our President of the Healthcare Division started as a PTA. Our Director of Clinical Management started as a field PT. Our VP of Senior Services started as a private duty nurse.

What you need

  • Degree in physical therapy from a CAPTE-accredited program
  • Current Ohio PT license
  • Valid driver’s license, reliable transportation, and auto insurance that meets our driving record standards

Nice to have

  • Home health or geriatric experience, including OASIS documentation
