Licensed Practical Nurse (LPN) – Home Health/Hospice 
Location: Central & North Oklahoma City, OK
Compensation: $32–$36 per visit
Schedule: Monday-Friday, Full-Time
Benefits: Full benefits package + mileage reimbursement
About the Role
We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our Home Health team in Central & North OKC. The ideal candidate will have at least 3 years of nursing experience and a passion for providing high-quality, patient-centered care in the home setting. This role offers flexibility, autonomy, and the opportunity to build meaningful relationships with patients and families.
Qualifications
  • Current Licensed Practical Nurse (LPN) license in the state of Oklahoma.
  • Minimum of 3 years of nursing experience (home health experience REQUIRED).
  • Strong clinical assessment and documentation skills.
  • Reliable transportation and a valid driver’s license.
  • Compassionate, dependable, and committed to providing exceptional patient care.
Responsibilities
  • Provide direct nursing care to patients in their homes under the supervision of an RN and in accordance with the plan of care.
  • Conduct patient assessments, monitor vital signs, and accurately document progress.
  • Administer medications and treatments as prescribed.
  • Educate patients and families on care plans, medications, and health maintenance.
  • Communicate changes in patient condition promptly to the supervising RN and physician.
  • Ensure compliance with agency policies, procedures, and state/federal regulations.
  • Travel to patient homes within Central OKC (mileage reimbursement provided).
