Job Summary:
We are seeking a compassionate, reliable Certified Hospice Aide to provide in-home care to patients throughout our service area. This position requires travel to patients' homes to provide personal care and support to individuals receiving home health or end-of-life hospice services. If you have a heart for caregiving and are comfortable working independently in a variety of home settings, we would love to hear from you.

Qualifications:

Current certification as a Home Health Aide (CHHA)

Minimum of 3 years of experience in home health or hospice care required

Current CPR certification

Valid driver’s license, clean driving record, and reliable transportation required

Willingness to travel
Key Responsibilities:

Travel to patient residences to provide personal care services, including bathing, grooming, dressing, toileting, and mobility assistance

Offer emotional support and companionship to patients and their families

Assist with light housekeeping, meal prep, and medication reminders

Observe and report any changes in patient condition to the care team

Follow individualized care plans under the supervision of a nurse or case manager

Maintain accurate documentation of services provided

Adhere to all infection control, safety, and confidentiality protocols
